Healthcare in the United States has changed. There have been scientific and medical advances and a great expansion of our knowledge base. There have been advances in diagnostic testing and treatment options. These are wonderful tools. However, the existence of these tools doesn't result in better care, or a better outcome, unless they are correctly used.


The best possible care is only achieved when doctors listen to their patients, ask the right questions, listen again for their responses, and know what to do with the information.


Unfortunately, the timely availability of the doctor and his/her attentiveness to patient needs have been compromised by some of the so-called advances in health care delivery. A dialogue between patient and physician does not easily occur if the patient cannot reach the doctor with questions and concerns. Nor can this dialogue occur if the physician is distracted with volume care.

About The Practice

About Dr. Horowitz

Dr. Horowitz attended Brandeis University and received his medical degree from New York University. He completed his internship and residency at Long Island Jewish Hospital. This was followed by a 2-year fellowship in general internal medicine at Georgetown University, Washington, DC. 


From 1983 - 1985 Dr. Horowitz was a full-time faculty member at Stony Brook University School of Medicine where he became an Assistant Professor of Medicine while participating in teaching, research, and patient care. He has been in private practice in Smithtown since 1985. In 2010 he established a solo concierge practice.

What if I need to see a specialist?

Dr. Horowitz coordinates your care with specialists and other providers. By doing so, he provides the specialist with targeted information about your specific issues. This helps the specialist focus on your needs and helps you get the most out of these encounters. 

Is there a fee for this concierge practice?

The annual concierge fee of $1,650 is paid directly to Dr. Horowitz by the patient. Payments may be made annually, quarterly, or monthly. A discount is offered for family members who also join the practice.

How does concierge practice work with private insurance?

The annual fee for concierge medicine services is not covered by medical insurance plans. That said, the practice works with your insurance company to bill for your medical care in the usual manner.

Dr. Horowitz is affiliated with Saint Catherine of Siena Hospital in Smithtown and Stony Brook University Medical Center. However, he has no obligation to any particular healthcare system. If a specialist or hospital-based care is required Dr. Horowitz will guide you to the specialist or healthcare center that best meets your needs
